 Travel Reimbursement

For the December 2017 Caravan Health Accountable Care Symposium, only AIM funded ACOs are eligible for travel reimbursement. For more information, CLICK HERE.

Starting January 1, 2017, CMS updated their requirements for the reporting of ACO related expenses and eligible reimbursements. Travel expense for ACO related activities is an eligible expense to be reimbursed through the updated ACO Reimbursement policy (this will count toward your community’s quarterly eligible reimbursement maximum). Please follow your individual community’s internal guidelines for specific travel accommodation rules and questions.

For your reference, please find the Caravan Health Excel template that will serve as a cover page for all reimbursement submissions HERE. To submit for reimbursement following your travel, please send the completed Caravan Health Excel template and all supporting documents to ACOreimbursements@caravanhealth.com . Please note, all travel expenses must be submitted no later than 10 days following the end of the calendar quarter in which they were paid. [Example: Travel to the April 2017 Quality Improvement Workshop must be submitted for reimbursement no later than July 10, 2017.]

Driving – Follow airport signs for 24th Street North (but do not exit 24th Street North). Go to State Route 51 North; follow 51 North to the Glendale Ave. exit.
